Dental Product Report Interview with Tais Clausen

Hello!

Well I am back from Toronto, Ontario Canada, where I had the pleasure of working along side some truly amazing and gifted people that this industry has to offer. As you may have read previously on my blog last week I was representing Kastle Mills along side Lee Cain, RDT and Kastle Mills 3Shape Certified Trainer as we joined the guys from DLA (Larry, Allan, and David) to present and introduce the 3Shape TRIOS for the first time in Canada! A lucky group of Dentists from the Greater Toronto area joined us as we reviewed the technology, benefits of digital impressions, and did some hands on scanning with the TRIOS live in action! It was a great evening and I was surrounded by wonderful people, thank you to Lee for being so helpful, knowledagable, and supporative, it was such a great experience working so closley with you!
So now I am back in Kelowna and gearing up for the 3Shape TRIOS second reveal this Thursday and Friday at the Thopmson Okanagan Dental Society 2012 convention and boy am I EXCITED! The team at Kastle Mills will be buzzing with enthausasiam for the next few days as we pull together and set up the most sleek and sophiscated exhbitors booth that you have ever seen, as we will have the 3Shape TRIOS there as the focal point complimenting our look!

3Shape Trios Study Club Presentation!

WOW... What a night!



First off I would like to thank Larry, Allan, and David for arranging this evening for the first debut in Canada of the 3Shape TRIOS and to celebrate the official release of this new ground breaking technology.
The dinner presentation was held at Santorini Restaurant in Richmond Hill, Ontario. Approximately 22 Dentists from the Greater Toronto Area joined as we discussed the features and highlights of the Trios, digital impressions vs. conventional impressions, the advancements and benefits of the technology compared to other Intraoral Scanners on the market today, the value it will bring to Dentists treating their patients ,and the leaps and bounds that the Trios will make for you and your team with implementation into your office and daily routine. Not to mention a live demonstration and hands on time with the Trios live in action!

Case Two: 3Shape Trios Model-less Workflow Validation

(Note we also processed precision Trios Digital Models for this case as part of the regular dentist workflow)

e. Max Press & ZirCules Crowns- both tried in prior to cememtation and both done completey model-less

Insert process:
Both units tried in prior to.
Both the e.Max Press and ZirCules had snug interproximal contacts. Dr. Brent C took an extra fine diamond finishing bur and lightly feathered the 35 distal and 37 mesial contacts.
The ZirCules Crown had perfect margins once seated, retention was good, the contacts were nice and snug made a nice snap with the floss, the occlusion was flawless - positive contact throughout and no adjusments required.
The e.Max Press had a slight over extension on the buccal margin, occlusion was slightly off, interproximal contacts good but slightly adjusted as noted above. This could very well be related to the fabrication process due to the conventional pressing method with expansion and contraction of the pressed ceramic there is a variable here that can definiley alter the fit of the restoration. Unlike the ZirCules which is a complete digitized process.



ZirCules margins were perfect! Dr. Brent C. simply put it as ' Best margins I have ever seen from a digital scanner! - He was excited about he 3Shape TRIOS and the ability to have multiple workflows one being model-less.

This case was scanned on the afternoon of October 2nd at 1pm, received by the lab in 2 minutes and 45 seconds, designed on 3Shape , milled on Roland DWX 50 5 Axis ( wax for e.Max burn and Zirconia for ZirCules), burned and pressed for e.Max process, sintered, stained, glazed, then shipped from Saskatchewan.
This case was then inserted the morning of October 5th at 9am ... turnaround times like never before!

On Sept 27th Dr. Brent C. took digital impressions with the3Shape trios- maxillary arch scan in 1 minute and 24 seconds with 338 3D images captured, Mandibular arch scan in 2 minutes 15 seconds with 528 3D images captured, and a 16 second bite reg with 70 3D images captured. (Pretty good for only his second time using the 3Shape Trios!)
On October 4th we seated one of two restorations (e.Max Press & ZirCules that were made for this case using a model-less workflow. These cases were designed on 3Shape D700, milled with Roland DWX 50 5 Axis/ burned out and pressed for e.Max, sintered, stained, glazed, and shipped half way across Canada in 4 days!

Guess which one we seated?!

e. Max Press:
Although the e.Max press was slightly less anatomical the margins were spot on and it seated perfectly! Snug interproximal contacts with light occlusal contact yet positive throughout the mouth - no adjustments were required!
(ZirCules shade was slightly off and patient preferred aesthetics of e.Max Press; however margins were perfect.)

Insert of 36 Zircules model-less workflow with 3Shape TRIOS

Throughout the last week Dr. Brent C. and his team scanned 2 cases with the 3Shape Trios. On Friday October 4th we were in clinic seating the restorations made for the 2 cases. From each scan we requested 2 crowns to be made; one from full contour monolithic Zirconia (ZirCules) and the other from e.Max Press. Due to the nature of both being a non layered product Dr. Brent C. opted out of the model fabrication process as they are not necessary.

Both restorations were tried in to check contacts, occlusion, and marginal fit. Both the e.Max Press and ZirCules had a snug interproximal fit; a light feathering approach on the interproximal contacts was done with an extra fine diamond bur to lighten up the contact.
ZirCules - a complete automated process, scanned digitally with the 3Shape TRIOS, designed digitally on the 3Shape D700, and milled with the Roland DWX 50 5 Axis mill. This restoration arrived at the lab in 2 minutes 45.5 seconds after the scan was sent. Witin 25 minutes it had been designed and in the mill for fabrication, it was sintered over night and shipped out the next morning!
The ZirCules restoration was prefect! The contacts were snug, the occlusion was ideal and no adjustment was required, and the marginal fit was precise, the explorer just glided over the margin onto the natural tooth surface.
e.Max Press -a digital/conventional hybrid if you will . This restoration was made using the same scan from the 3Shape TRIOS and the same design that was done on the 3Shape D700, however once designed a wax pattern was milled on the Roland DWX 50 5 Axis, invested, then burned for the ingot to be pressed. Due to the press process e.Max Press is not a completely digitized and introduces a few variables that can affect the fit of a restoration when compared to a monothlic restoration like full contour zirconia which is not a pressable material.
The e.Max press had ideal contacts however the marginal fit lacked as there was an over extension on the buccal margin and the occlusion was high. Due to this Dr. Brent opted to insert the ZirCules over the e.Max Press. ZirCules was inserted on the 36 using Relyx Unicem2 TR. Prior to insertion the restoration and the prep was cleansed with pumice and alcohol pellet to avoid contamination and ensure a strong cememtation.

In this interview article, 3Shape’s President and CEO Flemming Thorup answers some key questions about 3Shape, CAD/CAM technologies and the dental industry. Read some of his insightful views about the digital arena -and how he believes technology can help advance business in both labs and clinics.

01. 3Shape has a reputation for providing a continuous flow of new CAD/CAM solutions. How do you achieve this? Today we employ more than 130 developers along with experienced dental technicians and dentists, and we maintain close cooperation with dental professionals and labs around the world where new ideas are born and our solutions are evaluated in actual lab and clinical work.

02. 3Shape develops and delivers both CAD software and 3D scanners. Is there a reason? 3D scanners and design software integrated into one system give many advantages in terms of system performance, smooth workflows and thus the overall speed for completing cases. 3Shape’s scanners capture information such as texture data that the software is specifically designed to utilize for better designs. With solutions such as TRIOS and Communicate™, our integration concepts go beyond the lab’s system. 3Shape also develops its solutions to support smooth integration over complete workflows - starting from clinics, through the lab, and ending with manufacturing in milling centers. That is why we maintain our principles of openness and flexible interfacing in all our system components.

03. What type of lab is best suited for 3Shape scanners? Labs of any size can benefit from CAD/CAM because CAD/CAM systems can be matched to a lab’s specific business models making them good investments. 3Shape knows that labs differ, both in terms of size and the range of services they provide. That is why 3Shape offers both entry level and more advanced solutions for both scanners and software. We see a rapid development of new materials for prosthetic restorations and believe that CAD/CAM offers the best way to take full advantage of these while opening up new business opportunities. In our opinion, all labs can benefit from new digital technologies.

04. How does 3Shape help labs and clinics stay up to date? 3Shape believes that a dynamic and continuously up-to-date system should be a natural trait of any CAD /CAM solution for labs. We include unlimited upgrading as an integral part of our LAB care™ package and customer promise. For labs this means they can automatically and regularly empower their system with the latest technologies; their system represents a future-secure investment—it stays alive for years to come, growing stronger and more competitive every year. 05. Where do you see the largest potential for growth in CAD/CAM dentistry? We see potential both in the high-end sophisticated solutions like implant bars and major restorative constructions as well as in the design and digital production of Removable Partials. We also see growth in general business areas. Many labs that provide supplementary services for their dentists are winning territory in relation to production-only labs. One of 3Shape’s missions is to provide service features that help labs help their dentists. We have already developed digital temporaries, the model builder, virtual diagnostic wax-ups, online communication tools, and we have many more service tools in the coming release.

06. With TRIOS, 3Shape recently entered the clinic-side scanning market. How does this move benefit dentistry as a whole? Digitizing the process efficiently at the clinic with an intraoral scanner is a natural step in simplifying the workflow between the dentist and the lab. With the right approach, this technology brings huge advantages to both dentists and labs. While developing TRIOS, we focused largely on providing accurate impressions and improving communication between labs and dentist. TRIOS® output arrives directly in the lab’s CAD/CAM system. The system includes efficient communication and visualization tools, allowing labs and dentists to discuss cases while viewing the 3D model. The accuracy of the digital impression helps labs provide optimal results and additionally offer dentists new digital services while providing the full scope of dental indications and technician expertise. The TRIOS solution is designed to strengthen the ties between technicians and dentists, and strengthen the profitability of both clinic and lab services.

07. How do you see the future of the market and your own systems? We foresee continued development by the material manufacturers to capitalize even more on the advantages of digitized dentistry, along with software integration of different image technologies, design and production processes, new lab-dentist service tools, communication, training, and enhanced information sharing. Intuitive software and features like texture scanning, that utilize or mimic manual techniques in digital workflows, will help increase the use of CAD/CAM. We will continue to create solutions that directly answer challenges within the industry so that both labs and clinics can compete and prosper.
