We wanted to created a system that incorporated all the advantages and eliminate all the drawbacks of the existing systems. Our solution really needed to be faster, easier, more accurate and more reliable,” Deichmann said.

At the opening day of IDS 2011, 3Shape launched its newest achievement, the TRIOS intra-oral scanning solution, which aims to revolutionise the dental practice. The 3Shape booth was literally flooded with dentists trying to get a glimpse of sleek and elegantly designed scanner. One of the TRIOS 3-D scanner’s notable features is that it does not require dentists to apply spray or powder to coat the patient’s teeth, making scanning an easy, fast and comfortable process that does not ruin scan accuracy by adding material to teeth surfaces. In addition, it can scan any materials, such as metals, semi-transparent materials and skin. It only requires minimal amount of training for use in clinical practice. The scanner captures over 3,000 2-D images per second, which is 100 times faster than a conventional video camera. Dentists who witnessed the presentations at IDS stated that the ”impression-free” dental practice seems to be just around the corner.

An open communication interface allows dentists to send the scanned data via the Internet directly to the laboratory of their choice, where the technician can start designing the restoration immediately using 3Shape Dental System software or the appropriate interface to third party software. The TRIOS communication software also includes a tool to visualise the technician’s solutions for the patient, for example on an iPad, while the patient is still in the chair, which is especially important for anterior cases. The system is designed to give dentists high-quality restorations and treat more patients rather than spending time and money on chairside milling. It handles a wide range of indications and produces quality 3-D data that can easily be realised by any laboratory.

Generally, digital data is controllable, predictable and available any time by requiring only minimal space. This guarantees that the dentist owns and is able to use patient data without limitations and can potentially export virtual setups to other systems, such as for appliance manufacturing. Surprisingly, 3Shape is the only major dental company that goes by a non-exclusive basis. All products are designed as plug-and-play solutions and feature open interfaces for connection to third-party applications.

We have been working on a complete protocol for full arch screw retained full contour zirconia restorations.
This protocol includes all dental laboratory and clinical steps. As you can see we have come a long way and are now rolling it out.
The photos are of an actual case we recently completed using this process. The case is CAP FZ full contour zirconia except for the pink gum area.
As a result we are expecting long term success, as zirconia is virtually indestructible at 1100 MPa flexure strength. The process uses the CAP Complete system and includes a PMMA screw retained provisional designed in 3Shape. Followed with a final restoration that is milled in a Roland DWX50 5-axis mill using SUM 3D CAM, CAP milling strategies, and the highly esthetic CAP FZ zirconia. CAP NT Trading’s 2-CONnect abutments, titanium cylinders, and screws are used for connection to the implant.

Will, or when? The short answer to “will” is of course (in my opinion) a resounding “yes!” But the real question in my mind is “when?”
Digital impressions offer many advantages, more obvious perhaps for the patient initially, but benefits for the dentist and the technician are also highly significant. The technology is here, as can be seen from the ever-expanding array of commercial products available. The cost and the acceptance time needed for the ongoing developing technology will, however, hold back the full adoption of the technology for many practitioners for some time to come.
We have, of course, had digital impressioning with CAD/CAM systems for years. Although pioneering versions are here, what remains to be refined and accepted by the profession is the full-arch impression. The digital full-arch impression would eliminate many steps in making impressions, such as tray selection, dispensing and set up of materials, and disinfection and shipping of impressions that are subject to distortion. Such technology would eliminate patient gagging during impression taking. Digital files would allow for rapid communication with the laboratory, and a record that does not distort or deteriorate with time. “Re-pouring” the impression can be done with equal accuracy to the original. Initial data suggest that remake rates will plunge with digital technology—something that all stakeholders—clinicians, laboratory technicians, and patients—will welcome.

It is, of course, folly to try to predict when the life span of a material or a technique will gain full acceptance, or run its course. While patients and dentists alike will welcome the digitization of the impressioning step, the old ways from compound or rubber base to polyvinyl siloxane will only die out slowly (just look at dental amalgam). It is the younger, more flexible, and adaptive early-adopters who will be enamored by, and who will work to overcome, the early problems with digital impression taking, which will become the standard of care.

Interesting read: Breath analysis could diagnose colorectal cancer

BARI, Italy: Colorectal cancer, commonly known as colon cancer or bowel cancer, has one of the highest mortality rates of all cancer types. Yet no effective screening tool exists for the detection of this cancer. Italian researchers have now investigated a way to diagnose colorectal cancer by testing patients’ breath for chemicals linked to the disease.
In their study, researchers from the University of Bari investigated two groups of participants, 37 patients with diagnosed colorectal cancer and 41 healthy controls. Their exhaled breath was collected in a gas-sampling bag and evaluated for volatile organic compounds (VOCs) that could be red flags for cancer. According to the researchers, tumour growth involves several metabolic changes, leading to the production of specific compounds that can be detected in exhaled breath and thus analysis of the VOCs linked to cancer is a new frontier in cancer screening.
The researchers identified 15 of the 58 specific VOCs that are associated with colorectal cancer, and were able to distinguish with over 75 per cent accuracy which patients had cancer and which were healthy. Among the cancer patients, the team correctly identified 19 as having colorectal tumours.
“Breath VOC analysis appears to have potential clinical application in colorectal cancer screening, although further studies are required to confirm its reliability in heterogeneous clinical settings,” the researchers concluded.
The study, titled “Exhaled volatile organic compounds identify patients with colorectal cancer”, was published online ahead of print in the British Journal of Surgery.

Will, or when? The short answer to “will” is of course (in my opinion) a resounding “yes!” But the real question in my mind is “when?”
Digital impressions offer many advantages, more obvious perhaps for the patient initially, but benefits for the dentist and the technician are also highly significant. The technology is here, as can be seen from the ever-expanding array of commercial products available. The cost and the acceptance time needed for the ongoing developing technology will, however, hold back the full adoption of the technology for many practitioners for some time to come.
We have, of course, had digital impressioning with CAD/CAM systems for years. Although pioneering versions are here, what remains to be refined and accepted by the profession is the full-arch impression. The digital full-arch impression would eliminate many steps in making impressions, such as tray selection, dispensing and set up of materials, and disinfection and shipping of impressions that are subject to distortion. Such technology would eliminate patient gagging during impression taking. Digital files would allow for rapid communication with the laboratory, and a record that does not distort or deteriorate with time. “Re-pouring” the impression can be done with equal accuracy to the original. Initial data suggest that remake rates will plunge with digital technology—something that all stakeholders—clinicians, laboratory technicians, and patients—will welcome.

It is, of course, folly to try to predict when the life span of a material or a technique will gain full acceptance, or run its course. While patients and dentists alike will welcome the digitization of the impressioning step, the old ways from compound or rubber base to polyvinyl siloxane will only die out slowly (just look at dental amalgam). It is the younger, more flexible, and adaptive early-adopters who will be enamored by, and who will work to overcome, the early problems with digital impression taking, which will become the standard of care.

Holiday Spirit

I have had quite the week of unfortunate circumstances with remarkable outcomes that really make me feel that the spirit of the season really does exist! One in particular happened on Wednesday as I was traveling from the Kootenay Region of British Columbia back to the Okanagan Valley. I got a flat! It couldn't have happened in a worse location, on an "S" turn along the lake, not an ideal situation but what do you do. Called CAA they said a truck can be out to change my tire within an hour to an hour an a half.... eeek! I thanked the operator for her time and said I'm pretty sure I can do it myself within 30minutes and where I'm stopped I don't exactly want to be waiting long.

SO ... out comes my 3Shape TRIOS, my luggage, my safety kit, and everything else I had in the back of my vehicle. Open up the compartment, get out my jack, struggle with my spare tire and kick it out of the back ...everything accounted for, I AM READY!
It's funny being a woman 'stranded' so to speak, lots of people slowed down to look, honk and wave as the smile and drive on by but no one looked the least bit concerned or stopped to help. So I set up my jack, jack up my driver's side front end, take off my lug-nuts, and try my hardest to take my tire off the hub ... wouldn't budge ... gave it a few kicks and still wouldn't budge!

As I'm kicking my tire trying to loosen it off the hub and trying not to lose it this beautiful white Audi A4 drives past, brakes, pulls a U Turn and stops. YAY! A wonderful man named Kelly comes to my rescue. He laughed, gave the tire a good kick, got it off my hub, swapped it out and put my spare back on and I was good to go! Not only did Kelly make my day, he also made me more aware of others. Kelly didn't want anything for his time, he just smiled and shook my hand gave me a hug and told me to have a wonderful day.

Now luckily for Kelly during our conversation as he was putting on my spare I was able to find out where he works. Kelly is a foreman at a local lumber mill, Kelly loves to dirt bike, snowboard, and hang out with his friends. And Kelly should be receiving a very nice thank you package at work for taking the time to be aware, be present, and notice his surroundings. To show concern and take time out of his day to stop and help a stranger. I myself will be paying this forward.
Over the Holiday season please take time, be aware not only of your surroundings but of yourself, stay grounded, and pay it forward. Pick up a tab, buy a coffee, give a smile or a hug, let someone pass you or go ahead in line, whatever you do please do it with gratitude, awareness, and peace...a smile wouldn't hurt either.
